Taking steps to minimize the risk and spread of COVID-19 (or any other disease) is a responsibility shared by every member of the Montclair State community. Your responses to these questions will help you screen for symptoms and exposure to COVID-19 to maintain a safe workplace and campus. The information collected through this self-assessment will be used solely for the limited purpose of providing you support, to enable contact tracing (in the event of a positive or presumed case of COVID-19) and sanitization of areas as may be required by OSHA. By clicking on the box below, you agree that your responses are provided voluntarily. Your responses to the questions will be kept in a separate confidential file in accordance with federal and state laws. Responses to the questions that include the identity of an employee may be accessed only by HR/Benefits and Occupational Health, and will not be shared with administration or coworkers. Responses to the questions that reveal the identity of a student may be accessed only by the University Health Center and will not be shared with other students. You also agree that responses may be shared with public health authorities and those engaged in contact tracing in response to a report of a positive or presumed case of COVID-19. The identity of individuals who have completed Hawk Check (but not their responses to questions) may be shared with administrators in order to verify and enforce the use of Hawk Check.
